Menarche is the first menstrual bleeding experienced by a girl and marks one of the most important milestones in female growth and development. It is a significant event during adolescence, indicating that the reproductive system has matured sufficiently to begin the menstrual cycle. Menarche usually occurs during puberty as a result of hormonal changes that prepare the female body for reproduction. Although the occurrence of menarche does not necessarily indicate complete physical or emotional maturity, it is an important indicator of normal growth and development. From the perspective of community nutrition, understanding menarche is essential because nutritional status, health, environmental conditions, and socioeconomic factors have a direct influence on the age at which menarche occurs and the health of adolescent girls. The term menarche is derived from the Greek words "men," meaning month, and "arche," meaning beginning. It refers to the first menstrual period in a female. Menarche is the culmination of a series of developmental events that begin with puberty. Before menarche, girls experience breast development (thelarche), growth of pubic and axillary hair, rapid increase in height, body composition changes, and maturation of reproductive organs. These changes occur under the influence of hormones secreted by the hypothalamus, pituitary gland, and ovaries. The average age of menarche varies among populations but generally ranges between 10 and 16 years, with the average being around 12 to 13 years. The age of onset differs according to genetic, nutritional, environmental, climatic, socioeconomic, and ethnic factors. Improved nutrition and better living conditions have contributed to a gradual decline in the average age of menarche in many countries over the past century. Puberty begins when the hypothalamus starts secreting Gonadotropin-Releasing Hormone (GnRH). This hormone stimulates the anterior pituitary gland to release Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH). These hormones act on the ovaries, promoting follicular development and secretion of estrogen. Estrogen is responsible for the development of secondary sexual characteristics such as breast enlargement, widening of the pelvis, increased deposition of body fat, and maturation of the uterus and vagina. Eventually, the uterine lining becomes sufficiently developed, resulting in the first menstrual bleeding known as menarche. The menstrual cycle involves cyclic hormonal changes that prepare the uterus for possible pregnancy. The first few years after menarche are often characterized by irregular menstrual cycles because ovulation may not occur regularly. Over time, hormonal regulation becomes established, leading to regular menstrual cycles. Nutrition plays a fundamental role in determining the timing of menarche. Adequate intake of calories, proteins, vitamins, and minerals supports normal growth and hormonal function. Girls with good nutritional status generally attain menarche earlier than those suffering from chronic undernutrition. Body fat is particularly important because sufficient fat stores are necessary for estrogen production.
