b) A 67 years old female patient presented with complaints of fever, headache, and vomiting for the past 1 week, photophobia & altered sensorium for the past 3 days. On examination the patient was conscious, Pulse: 110 beats/min, Blood pressure: 100/70 mmHg & temperature 100.6 oF. CNS examination: neck stiffness +, Brudzinski’s and Kernig’s signs of the neck and legs are also present. Laboratory investigations revealed Opening CSF pressure is elevated to 400 mm H2O. The CSF appearance is turbid showing presence of protein. The white blood cell count is elevated to 3400 cells/mm3. MRI scan has noted minimal diffuse leptomeningial enhancement.

These signs, however, may not always be as prominent in elderly patients, as their response to infections is often blunted, and the immune system may not mount a robust inflammatory response.
