HIV/AIDS is a global public health issue, but its distribution is uneven across different regions of the world. Some regions carry a disproportionately high burden of infection, while others have relatively low prevalence rates. The regional distribution of HIV/AIDS is influenced by a combination of social, economic, cultural, political, and health system–related factors. Sub-Saharan Africa is the most severely affected region in the world. It accounts for the majority of people living with HIV and a significant proportion of new infections and AIDS-related deaths. Countries in Eastern and Southern Africa, such as South Africa, Botswana, Eswatini, and Lesotho, have particularly high prevalence rates. Several factors contribute to this situation. Widespread poverty limits access to education, healthcare, and preventive services. Gender inequality plays a major role, as women and girls are often unable to negotiate safe sexual practices and are biologically more vulnerable to infection. High rates of sexually transmitted infections, low levels of male circumcision in certain areas, migration for labor, and stigma associated with testing and treatment also increase vulnerability. Weak health systems in the past limited early diagnosis and access to antiretroviral therapy, allowing the epidemic to spread widely before effective control measures were implemented.
