Projection is a psychological defense mechanism in which an individual unconsciously attributes their own unacceptable thoughts, feelings, or impulses to another person. In mental health nursing, understanding projection is essential because it frequently appears in patients with anxiety disorders, personality disorders, psychotic conditions, and even in everyday stress reactions. For example, a patient who harbors aggressive impulses but finds them unacceptable may accuse others of being hostile or threatening toward them. Projection serves as a way of protecting the ego from anxiety and internal conflict by externalizing the source of distress. It helps the person avoid facing the real emotional reasons behind their actions. In mental health settings, patients may rationalise substance abuse by saying they “need alcohol to sleep” or justify aggression by claiming they were “provoked.” Rationalisation is commonly observed in depression, substance use disorders, personality disorders, and adjustment problems. While it may reduce guilt and anxiety temporarily, it prevents insight and personal growth.
