Human factors analysis and classification system (HFACS) is a framework used to analyze and classify human error in aviation and other high-risk industries. It was developed in the late 1990s by Dr. Douglas A. Wiegmann and Dr. Scott A. Shappell at the University of Illinois at Urbana-Champaign, and it has since been widely adopted as a tool for investigating accidents and incidents.

The HFACS framework is based on the premise that human error is not solely caused by the actions of individuals, but rather by a complex interaction of factors that can be classified into four categories: organizational, supervisory, environmental, and individual. These categories are further divided into 12 subcategories, which are used to classify the various factors that contribute to human error.
