Describe pressure sore and how it can be prevented

A pressure sore, also known as a pressure ulcer, decubitus ulcer, or bed sore, is a localized injury to the skin and underlying tissue, typically over a bony prominence, due to prolonged pressure or friction. These sores are most commonly found in individuals who are bedridden, wheelchair-bound, or have limited mobility, such as elderly patients in long-term care facilities. Pressure sores develop when sustained pressure compresses blood vessels in the affected area, reducing blood flow to the tissues. Without adequate blood circulation, the tissues become starved of oxygen and nutrients, leading to tissue damage and, in severe cases, necrosis.

The development of pressure sores can be classified into four stages, based on the severity of the injury:
